The Brent Winter Ward runs throughout
the year at the Willesden Green Centre for Health and Care and offers support
to vulnerable people leaving hospital.
Thames Reach staff provide housing
advice to people using the centre and form part of a
specialist team including doctors, occupational therapists, physiotherapists,
psychologists, speech therapists and social workers.
The majority of patients based in the
hospital are vulnerable elderly or those who suffered major neurological issues
such as a stoke, aneurism or brain injury.
Thames Reach staff work with patients
who have either became homeless as a result of hospital admission or are being
at risk of losing their accommodation. Thames Reach offer support around
housing, welfare, debts, education as well as addressing further issues around
