Camden Transitions

Staff support people in their homes

Camden Transitions is a floating support service that enables people to make the transition from living in supported housing to moving into independent accommodation.


The service helps those with low-level mental health issues to move on with their lives. Staff visit people in their own homes, in hospital and in the local community to in an effort to support people in making the transition to independent living.  


People are offered specialist, transitional support for six months. This can be extended under certain circumstances, such as if the individual needs further support. The focus is on helping people integrate into the local community, develop the skills and confidence to get back into work and to help people follow their aspirations and set personal goals.


People are referred into the service via the Camden Community Mental Health Team.
