The Living Well Network Alliance is a unique partnership between health, adult social care and voluntary sector support delivering innovative mental health services for adults across Lambeth.  The LWN Alliance is responsible for all inpatient and community care services in the borough.

Funded through the Alliance, CLaSS is a 12-month pilot project which will improve outcomes for people with mental health needs receiving acute care in Lambeth.

The team will oversee transitions of care into the community for patients whose discharge from hospital has been delayed;  support the entire Mental Health System to improve practice in managing crisis.

You will be supporting people in the community to avoid admission as well as ensuring that people with more complex needs can be discharged from hospital in a timely manner.

Working within a multi-disciplinary team made up of a clinical lead, social workers, voluntary sector support workers and administrative staff. This post will give you the opportunity to build on your ability to carry out assessments, identify options, devise plans to achieve these and oversee a multi-agency problem solving approach with people who may have a history of difficulty maintaining accommodation while gaining experience in an exciting new service area

You will have:

– A commitment to supporting people to live in the least restrictive setting

– A high level of motivation

– Great communication and negotiation skills

– The ability to work effectively within a multi-disciplinary team

– Knowledge around practices relevant to achieving timely discharge
